www.chp.gov.hk/ceno) is subject to the following terms and conditions. CENO On-line is managed by the Centre for Health Protection of the Department of Health (referred to as "We" below). In entering our website, you as a user (referred to as "You" below) are accepting the following terms and conditions.
Terms and Conditions
For all users of CENO On-line
CENO On-line is maintained to facilitate notification of statutory notifiable diseases and other communicable diseases or conditions of public health concern. The access and use of CENO On-line featuring these terms and conditions constitutes your acceptance of these terms and conditions. They take effect from the date on which you first use this website. We may at any time revise the terms and conditions without notice. Please check regularly. Continued use of the website after a change has been made is your acceptance of the change.
You agree to use CENO On-line only for lawful purposes, and in a manner that does not infringe the rights of, or restrict or inhibit the use of CENO On-line by any third party. Such restriction or inhibition includes, without limitation, conduct which is unlawful, or which may harass or cause distress or inconvenience to any person, and the transmission of obscene or offensive content or disruption of normal flow of dialogue within the website.
We advise you to use digital signature and encryption if you choose to notify CENO by sending email to diseases@dh.gov.hk This is to protect patient's information from unauthorized interception or modification. You also have a responsibility to ensure laws are not broken when composing or forwarding e-mails and their contents.

For users of CENO On-line web-based notification system
The CENO On-line web-based notification system (referred to as "system" below) is located in a secure area of the website, accessible only through login by registered users.
By registering as a user of the system, you agree to additional terms and conditions as follows:
- You shall provide true and accurate personal information during first registration, provide updates in your profile when changes occur, and authorize CENO to verify your personal information. (Please also read the privacy statement.)
- You shall ensure the accuracy, integrity and quality of information you report in the system.
- You shall exercise reasonable care and diligence in keeping the login ID and password in secrecy, should not disclose the login ID or password to a third party, and ensure that you do not knowingly or accidentally share, provide or facilitate unauthorized use of them.
- If you forget your password and/or login ID, you shall be denied access to the system and records of your previous notifications, until you have provided CENO with adequate personal information and supporting documents to process your request for resumption of service.
- You shall inform CENO immediately if your login ID and password have been disclosed to a third party, lost or stolen, and unauthorized transactions may have been conducted.
Security-related Recommendations
- Make sure you are connected to the official CENO On-line website at www.chp.gov.hk/ceno before keying in any confidential data
- Do not follow CENO On-line hyperlinks embedded in email
- Type www.chp.gov.hk/ceno directly on the browser address bar or access via a bookmark
- Check for the locked padlock symbol at the bottom right hand corner of your screen
- Change your login password regularly
- CENO On-line should not be accessed from public-access workstations (e.g. PCs in cybercafé or public kiosk)
- When not in use, CENO On-line account should be logged out and all CENO On-line pages should be closed before leaving the PC or workstation
- PC or workstation you used for accessing CENO On-line should have the appropriate anti-virus program running with the latest virus definitions
- PC or workstation you used for accessing CENO On-line should have the appropriate latest security patches installed and applied
Privacy statement
Personal data provided by doctors to CENO On-line will be used for the following purposes:
- Verification of eligibility for services
- Management of CENO On-line user accounts
- Notification and investigation of statutory notifiable diseases and other diseases of public health concern
- Organization of activities related to epidemiological surveillance
- Preparation of statistics, carrying out research or teaching
Registration and provision of personal data for registration as a CENO On-line user are voluntary. If you do not accept the terms and conditions or do not provide adequate personal data, then the registration procedure cannot be completed and we cannot provide the web-based notification service to you.
The personal data you provide are mainly for use within the Department of Health, but they may also be disclosed to other Government bureaux, departments or organizations for the purposes mentioned above, if required. The data may only be disclosed to parties where you have given consent to such disclosure or where such disclosure is allowed under the Personal Data (Privacy) Ordinance.
Under the Personal Data (Privacy) Ordinance, you have the right to request access to, and to request correction of, your personal data. The CENO will handle your written request for personal data access in relation to CENO On-line registration and notification.
